Youth came to our ED on 9/4/25 from a group home after reportedly ingesting extra medications. It’s unclear how she got the medications, but her custodian does not want her returning to the home at this time. IP was initially recommended however she was declined from multiple IP units due to feeling there would be no benefit from IPMH and due to her acuity. She has since stabilized while waiting in the ED, and is now being recommended for discharge. This is this youth’s 7th time boarding in Fairview EDs. There is a tentative placement set up for the 22nd, looking for potential interim plans.

15-year-old female presenting to PrairieCare Inpatient Hospital as a step up from Newport RTC due to an increase in intrusive SI. She will not be returning to Newport per parent request due to SA and elopement while in programming. She has a history of depression, anxiety, ADHD, and PTSD. This is her 4th psychiatric inpatient hospitalization. She has also engaged in PHP and outpatient psychiatric services.

17-year-old female who presents to PrairieCare Inpatient Hospital due to suicide attempt with increased SI and SIB.
This is her 2nd psychiatric hospitalization. She has engaged in PHP and outpatient psychiatric services.
For RTC – she will need funding through Ramsey County.

15-year-old female history of depression and anxiety PTSD. Significant sexual abuse by immediate family members. Admitted to hospital after argument with grandma, who is guardian. Patient then tried to strangle herself by placing a 20-foot cell phone cord around her neck. Perpetrator of sexual abuse is out of jail on bail and has cut of ankle monitor. Significant long standing sexual abuse, trauma, and drugs from abuse.

Mom transferred to rights to dad 2 years ago through a CHIPS petition. McLeod county CPS has been working voluntarily with the family for awhile (CPS worker Daisy Vinkemeier 320-300-8724). Dad had been doing well up until recently when he relapsed. Dad was admitted to the hospital yesterday at the same time as patient due to medical issues. Due to this, McLeod County has placed patient on a health and welfare hold 8/28 until the team can get to court to take temporary custody of the patient. At this time there are not any kinship options for the pt.

Came to ED 8/29 when adoptive parents called 911 after pt posted a video to social media claiming her adoptive father was abusing her. Parents are refusing to let youth return home as they are scared of further false accusations. They are requesting CPS get involved and take custody of youth, as they no longer want to have it. Has some history of SIB in the past. Waiting for CPS to determine next steps.

She had been residing at Von Wald 180 Degrees since May. Big escalation on 8/19 and brought to ED. Unable to return to Von Wald; parents are guardians but unwilling to have her in their home (possibly homeless?) and want to terminate their parent rights. Mom is difficult to get in touch with and wants all communication through her attorney.

Was in metro area with adoptive parents. Placed at Youth Shelter in St. Cloud. Came to hospital. Shelter d/c’d her and refused to pick her up. Ramsey probation and SW already involved. Adoptive parents want to relinquish any rights to patient as aggression is focused on mom.

Recommending Level 5 QRTP.
11 year old female. Presented to PrairieCare Inpatient Hospital due to increased family conflict and SI. She has had attended PHP numerous times. Most recently she was at PrairieCare PHP and stepped up to inpatient due to an increase in safety concerns. The family and outpatient team were already pursuing level 5 QRTP although were running into funding barriers. She does have a current CMHCM with Anoka County. This youth will need funding from the county to support QRTP admission.
