Patient admitted to unit after ER boarding within the ER for an extended period as mom didn’t feel safe with him returning home. He has a chronic history of extreme emotional dysregulation with acute worsening since moving to the unit & significant head banging and aggression towards himself and others. He has a history of trauma that is likely impacting his presentation.

Patient presented to the ED after self-presenting to a police station reporting herself as a run away. Ran away from shelter for about one week and reports staying with friends. County is guardian and looking at shelter or kinship placement options.

Patient presented to the ER via EMS after being found outside at a local school’s playground intoxicated, hypothermic, and with an altered mental status with a concern for head trauma and a seizure. He was medically stabilized and admitted to the MHU. He had just completed residential CD treatment on 1/24/24 after continued use while in IOP with plans to transition to IOP again though he self injured via cutting on his arm, began to drink, and snuck out the window in the middle of the night.

The patient is very depressed with no motivation or desire for sobriety. He has a history of a previous suicide attempt. He does require residential CD treatment again and will need to transition straight from the hospital to treatment. .

Patient presented from aunt’s home where she has been in foster placement, however aunt unwilling to allow to return home following an altercation about her cell phone. Unable to return to foster placement and CPS does not have immediate options for placement alternatives. Presents with aggression primarily toward family. HX of abuse.

Patient presented from Aspen House due to suicidal ideation and aggression. He has stabilized and is at baseline level of functioning. Patient unable to return to Aspen House and does not have an alternative placement he can go to. CPS looking into long term placement options. Presents with aggression and suicidal ideation, elopement.

Patient presented to the ED from school reporting suicidal ideation. Is able to safety plan for discharge however mother not answering calls and unable to discharge related to this. Historically has been in crisis respite placement after mother refused home last fall from a hospital stay.

Case Description
15-year-old female with a history of depression, anxiety, ADHD, PTSD, as well as some behavioral concerns. Pt has had three hospitalizations in the past year, has done PHP, JDC, outpatient.
