She has been accepted to Gerard Academy. Waiting on an admission date.

admitted to Comprehensive Home Care (Respite Placement) for a short-term stay until he can be admitted to CABHH.

Discharged to crisis respite provider 6/6

Pt was initially in Fairview ED for 2 months, transferred to inpatient to board there for 1 month, was discharged to a foster placement for one week, and returned again to Fairview ED 6/4 due to behaviors, is again boarding in ED. Struggling with appropriate placement due to autism, limited verbal skills, and ongoing aggressive behaviors.

Update 6/5/24: Patient discharged from inpatient to home with outpatient services. County declined funding support for medical recommendation of QRTP. Patient was accepted to North Homes RTC and Avanti RTC with openings the first two weeks of June, although without funding support patient is unable to admit. 

Update 6/5/24: County funding approved for interim options. Patient has been accepted to Aspen House Shelter with expected admission end of week-early next week. Nexus East Bethel PRTF estimated intake moved to end of August.

Update 5/29/24: Accepted to Nexus East Bethel, PRTF funding approved, tentative admit now second week of June. SEEKING INTERIM PLAN for Respite, Shelter Plus, etc. Referred to Shelter Plus, will need county funding. Preplacement/JST scheduled for 6/3
