This youth presented to the hospital after eloping from her foster residence. She had been on the run for over a month and was brought in by EMS after attempting to enter a youth shelter. She was admitted to the hospital for medical needs and determined to be appropriate for discharge. The foster residence is unable to take her back at this time due to needed home repairs and working on increasing staffing from 1:1 to 2:1.

Admitted to hospital due to suicidal ideation, auditory & visual hallucinations. Pt presented after a suicide attempt via intentional overdose on her prescription methylphenidate.
Predisposing factors: chronic mental illness, extensive family history mental illness; early exposure to domestic violence, childhood trauma, seizure disorder. Precipitating factors/Stressors: family dynamic. Perpetuating factors: history of CPS involvement, chronic mental illness, family dynamic/stressors.

Pt has hx of several suicide attempts, chronic SI, impulsivity, substance use, and PTSD with dissociative symptoms. Pt has a long history of IPMH and two residential stays at Heartland Girls Ranch. She recently was at a dual treatment center but was discharged following a suicide attempt. We were initially looking for dual RTC, but we have expanded our search to find placement as pt continues to struggle with prolonged hospitalization.

Patient presented to the ER from residential facility in Wisconsin. He was discharged unsuccessfully from their program due to behaviors and acuity and transported here. Wrap around case manager is looking for placement. Patient’s mother does not feel able to have him home due to his behaviors, property destruction and eloping.

Youth came to our ED 7/16 after running from a youth shelter. The shelter will not allow her to return. Has a history of substance abuse, has been to at least 3 SUD residential treatment programs, including a cultural specific program in CA and discharged from all of them. No other SUD residential programs will accept her. History of elopement. Looking for placement to get her out of the ER. Currently no waiver, and has not been through County screening for MH RTC. Last SUD/comprehensive assessment 4/28 recommended residential and last DA 4/2 also recommended residential.

Youth came into the ED 6/26 after being discharged from the shelter program they were in. Reportedly were boarding in a different hospital last week. Currently accepted at RTC but admit isn’t until 7/10. Needing an interim plan. County has permanent guardianship. Has been denied by most shelter programs. Does have waiver funding but no waiver CM yet.

Youth came to the Grand Itasca ED 7/8 from a group home setting. They are unwilling to take him back. He is under custody of the County. Has been denied by programs primarily due to history of aggressive behavior. GH was able to complete SUD assessment 7/14 and waiting for that recommendation however pt is not currently agreeable to treatment. MNchoices was able to be completed 7/15. Is on MCCP crisis pool list. CPS is working on potential kinship placement and is meeting with that family today , 7/16. Remains on YSC list but is #8 so no likely bed for some time.

Youth came to ED 6/17 after altercation with siblings at home. Adoptive Mom is not allowing her to return currently. Has boarded in our ER 2 other times, in April 2026 and May 2025, returned to Mom both times previously. Has DA and approval for RTC, primarily looking at RTC options. Trying to get further medical records to support RTC referrals from Children’s. No MNChoices/waiver currently.

Youth came to our ED 5/31 from a waiver placement that issued immediate suspension/termination. County has custody. Due to his age and some behavioral concerns, placement is difficult to locate. This is his 3rd time boarding in our ER in 3 years. They are currently searching for waiver placements crisis respite and CRS, and he is on the list for YSC, along with several other youth. He’s been on MITH wait list for years and remains on MCCP list.

Youth came into Nexus with substance use concerns. He has not struggled with any urges to use since he was admitted. Brian struggles with noticing risky situations. He often talks about taking care of his mom who struggles with substance use (Meth) and her own mental health. Brian is fixated on going home to mom, but CPS is involved and Brian is not able to go home at this time.
