RCCP-M13-7590

He is new-to-country, arriving in June from The Gambia. He is English-speaking.

He has a record in the past month of trying to wander around – likely a cultural habit of wandering around the community, saying hi to people, just kinda hanging out. He wanders and then often returns within a few hours.

Current dx: Adjustment Disorder due to transitions related to moving to the US and living with mom.

Mid-October, he tried to force another male youth to perform oral sex on him. 5th Deg CSC charges pending. He has a pending Rule 20 competency eval & full psych eval.

He was in a CADI home where we contract for a crisis bed (no current waiver). Over the weekend, he punched 2 peers and was discharged.

Current VPA to allow placement authority to Ramsey County.

Goal is to return him to The Gambia.

Youth came into our ED on 7/20. Came straight from another hospital. Has previously been in hotel crisis respite however due to history of property destruction was not allowed to return to the hotel location and provider has not been able to identify another hotel that pt can go to. Is on waiver and team is working toward a single site home, however waiting for DHS to give provider their UMPI# before they can take pt, unclear how long that could be. Parents are refusing to take youth home in the interim.

Pt has behavioral concerns of elopement and some aggression. Has intellectual disability though it seems IQ testing has not been completed since he was 5, is mostly non-verbal. CPS is petitioning for custody. DD waiver is being opened, and crisis respite options are being explored. Reported that he is a “2 county kid” with both Ramsey and Hennepin involvement. Has a referral in for stabilization center however its unknown how long that list is or if he will be accepted.

Youth came to our ED 4/24 from a foster home that is refusing to let her return. Reported she has recently boarded in multiple other hospitals. County is currently seeking group home placement, in both therapeutic group home settings and CRS and crisis respite settings via the waiver. The last DA done earlier this month has a recommendation for OP services which the team does not agree with so they are also seeking an assessment program to determine appropriate level of care, North Homes denied, waiting to hear on Gerards assessment program.
