Client was referred to my program but we are unable to support his needs.

Has been to residential before for 6 months last year. Mom reported that when he got home he regressed back into old behviors including physical agression regularly; at times multiple times at day. He has also been to PHP before. He is currently at a respite home for the next 80 or so days. Mom reported insurance has been tricky and waitlists at programs willing to assess him have been long as well. History of eloping from school as well.

Child residential treatment since April 2026. After she had assaulted someone at the resident, she was brought to JDC. After JDC, she returned home.
Child presented originally to ED 8/2 via EMS after intentional ingestion. She was recommended IPMH after MHCA. No units were accepting of child; after psychiatry consultation, she was appropriate for discharge to home. Mother refused to discharge child to return to home. Extended family members not willing to have her stay with them at this time.

Pt was unable to play with balloons with peers and got upset, leading to dysregulations and ran away from group home. Pt was found by police and became physically aggressive towards others. Pt assaulted staff, assaulted sheriff deputies and broke windshields with rocks. She is unable to return to group home.

Pt came to us 4/23. Was residing with adoptive parent who is refusing to let her return. Pt turned 18 days before coming to us and is currently her own guardian, hearing for guardianship 4/30, will be a professional guardian agency. Adoptive Mom got custody in July 2025, waited 8 months for the MNchoices, done in Feb, and has since been waiting over 2 months for SMRT in order to get waiver. DD Case Manager (non-waiver) was just assigned 2 weeks ago. We were able to expedite SMRT due to being in the hospital and she is now certified as of 4/30, so moving forward with waiver. Looking for waiver placement, immediate and long term for this individual, perhaps a transition age program. Pt is vulnerable so typically shelters would not be an option. Still in High School currently.

The Guardian/Dakota Cty are planning for residential treatment for pt and she is indeed currently court ordered to remain hospitalized until this is located. She does not need inpatient level of care but here is no safe discharge location. History of sex trafficking and was recently picked up with a known trafficker. It was verbally reported that pt had a DA in July that recommended a level 6 placement and that CM was working on a facility in Arizona. Will ask for confirmation and documentation as hear from CM and request list of referrals, assist as able.

Youth came to our ED on 9/4/25 from a group home after reportedly ingesting extra medications. It’s unclear how she got the medications, but her custodian does not want her returning to the home at this time. IP was initially recommended however she was declined from multiple IP units due to feeling there would be no benefit from IPMH and due to her acuity. She has since stabilized while waiting in the ED, and is now being recommended for discharge. This is this youth’s 7th time boarding in Fairview EDs. There is a tentative placement set up for the 22nd, looking for potential interim plans.
