Psychiatric Diagnosis: Substance Abuse

Substance Abuse refers to problematic patterns of using and abusing drugs or alcohol, leading to negative consequences in a person’s life. It is a psychiatric diagnosis that may involve addiction or substance use disorders.

Pt has hx of several suicide attempts, chronic SI, impulsivity, substance use, and PTSD with dissociative symptoms. Pt has a long history of IPMH and two residential stays at Heartland Girls Ranch. She recently was at a dual treatment center but was discharged following a suicide attempt. We were initially looking for dual RTC, but we have expanded our search to find placement as pt continues to struggle with prolonged hospitalization.

Patient presented with Benadryl overdose requiring ICU stabilization. Unable to return home at this time as she requires residential SUD treatment. Struggling with obtaining facility call backs and finding locations that accept her age and insurance.

Youth came to our ED 7/16 after running from a youth shelter. The shelter will not allow her to return. Has a history of substance abuse, has been to at least 3 SUD residential treatment programs, including a cultural specific program in CA and discharged from all of them. No other SUD residential programs will accept her. History of elopement. Looking for placement to get her out of the ER. Currently no waiver, and has not been through County screening for MH RTC. Last SUD/comprehensive assessment 4/28 recommended residential and last DA 4/2 also recommended residential.

Youth came into our ED on 1/13/26 and was determined to be ready for discharge. Came from Grandparents who are refusing to take him home until he gets SUD treatment. He is unagreeable to SUD treatment at this time, willing to do OP. County is not funding shelter due to not having custody. Parent referred shelter referrals have been made however custodians refused the Bridge who were looking at accepting him. Has been to Lakeside Academy and Wings SUD treatment in the past and eloped from both. No waiver currently. FV Maplewood declined primarily due to pt being unagreeable to treatment, but also due to concerns of elopement and Grandparents being willing to get him from the ED. Has been declined by many other RTCs as well. Referral pending with Anthony Louis.

Currently in custody of St. Louis County. Ongoing CPS case management. Due to presenting incident that involved patient threatening SI and possibly threatening other family members in the home, current foster placement unwilling to have patient return.

MHF-M17-6251

Youth came to our ED via police on 10/28. Was previously in our IP unit, discharged to Nexus Gerard where he eloped several times. Pt is on a MI/CD commitment. Has substance use struggles ongoing but it’s reported that previous placement thought the MH was the primary concern. Has been denied by many SUD programs- Wings, Anthony Louis, Fairview, Oshki Manidoo and Ember in Iowa. Ideally CM wants pt in a secure setting due to history elopement, but has been denied by only secure setting in MN. Pt is newly 17. Any facility licensed by the DOC can’t be pursued due to commitment, and going out of state the commitment would not follow, so in state would be best.

Youth came to our ED 4/17 from group home, who are not allowing her to return. Stearns County has custody currently and there is a CPS worker as well as ICWA advocate. No waiver currently. Had a SUD assessment that recommends residential treatment and she was agreeable to that, however no acceptance at programs yet and wait lists for those also. CPS is exploring shelter and kinship options. Was denied by Red Lake shelter and Ain Duh Yung.

Patient presented to the ER with PD following being on run for one week from a trial home visit with biological parents, was in shelter prior to that. Drug of choice is meth. Initially presented with psychotic symptoms that have since cleared, which has resulted in patient being cleared by psychiatry.
