Admitted to hospital due to suicidal ideation, auditory & visual hallucinations. Pt presented after a suicide attempt via intentional overdose on her prescription methylphenidate.
Predisposing factors: chronic mental illness, extensive family history mental illness; early exposure to domestic violence, childhood trauma, seizure disorder. Precipitating factors/Stressors: family dynamic. Perpetuating factors: history of CPS involvement, chronic mental illness, family dynamic/stressors.

Admitted due to suicidal and homicidal ideation. Patient struggles with explosive outbursts when she does not get her way. Patient has a hx of calling CPS frequently alleging abuse against mom. Diabetes was managed with an insulin pump until recently, when the pump was removed for inpatient admission and she was transitioned to insulin injections. She reports that the pump was helpful in regulating her diabetes. She reports daily suicidal ideation, describing it as a persistent struggle, with plans to smother or strangle herself. She also experiences homicidal ideations, particularly directed toward her mother and sister.

Adopted adolescent that came from group home prior to presenting to the Emergency Department on 4/27/2026 with Law Enforcement due to increased aggression and agitation at group home. 4/26/26 had aggressive behavior at his group home physically assaulting two staff members. One staff sprained a wrist and the other staff required facial surgery following the incident. Earlier in the day he had made threats to stab teachers and staff at school. He reported he meant to strike is housemates but struck the staff instead. A few years ago patient had a previous episode of physical aggression toward caregivers and was hospitalized at that time.

Patient is a 16-year-old female presenting to the emergency department with concerns for psychiatric evaluation. Patient states that she is here because she has had suicidal thoughts. She states that she “acted out” at home today. She reports that she was recently hospitalized at Prairie care for a month and 1 week. She states that she was “kicked out of residential” because of frequent outburst. Patient states that she did not feel as though she was getting the help that she needed. She reports that she felt as though the workers there were talking about her. She states that today she again felt more suicidal. She states that she cut her arm with the top of a container. Denies any recreational drug or alcohol use. She denies other concerns at this time.

Previous psychiatric diagnoses include PTSD, RAD, Dysthymia, GAD and ADHD. She has had 2 previous psychiatric hospitalization(s). Most recent psychiatric hospitalization was 3/22/26 at Prairie Care, Altru health 2/12/26 for 13 days. Prairie Care in December 2025 due to behavioral escalation and SI crisis. . She admits current or past partial programs or residential programs. She was at Prairie Care Residential for one month. She had to leave due to her behaviors like hitting, screaming, biting and trying to kill herself. She denies psych testing. She admits to 1 previous suicide attempt(s): In December jumped in front of a car. She admits to engaging in self-injurious behavior. Last SIB Today cut her left inside forearm.

Patient presented to the ED after ingesting a button battery. Surgical intervention was needed to remove the battery. Safety plan competed to allow patient to return home with father. While at school the next day, she hid in the bathroom and broke open a calculator to swallow 2 AA batteries. Hospital admission needed until batteries passed the next day. Acute inpatient MH options explored with no accepting facilities. Remains admitted awaiting residential placement at this time. Father does not feel he can keep her safe at home due to the increased risk of swallowing objects.

Patient presented to the ETC from a local Youth Shelter. Patient is not able to return to the shelter. Patient is in the custody of Hennepin County. Patient has significant trauma that he has been able to truly process and understand.

Patient presented for overdose following rule enforcement at home. This is patient’s 5th inpatient hospitalization for suicide attempts/ideation. Suicidality appears to be in context to limit enforcement and a desire “to get back at mom.” Appears manipulative in nature. He attends most programming and has not displayed any emotional or behavioral dysregulation since admission.

Youth came into our ED 10/31 after an altercation in the car with Mom, on the way from being discharged from RTC after 6 months there. Mom reports she can not take her home. County would prefer higher level of care than RTC however she has been denied by Grafton, so only 1 potential option remains and referral was made. MNchoices is being completed today 11/5. Exploring all options currently. Has recent sexual exploitation history.

Presented to ETC after overdose on medication. Recent hospitalization at PrairieCare and overdosed after returning home. Has had 4 inpatient hospitalizations over the last year. Participated in partial hospitalization program after inpatient stay in March. Recommending residential hospitalization due to multiple inpatient hospital stays in the last year.
