Patient presented to PrairieCare Inpatient Hospital after suicide attempt by hanging on two consecutive days while at Avanti Center for Girls Residential. She has been at 3 different residential treatments (North Homes, Bar None Haven, and Avanti). This is her 4th psychiatric inpatient hospitalization.

Discharge Plan:
Establish QRTP/PRTF:
-Avanti (discharged d/t suicide attempts and elopement concerns)
-PrairieCare Residential (declined d/t suicide attempts and elopement concerns at Avanti)
-Grafton (referral sent)

Youth came to our ED 7/16 after running from a youth shelter. The shelter will not allow her to return. Has a history of substance abuse, has been to at least 3 SUD residential treatment programs, including a cultural specific program in CA and discharged from all of them. No other SUD residential programs will accept her. History of elopement. Looking for placement to get her out of the ER. Currently no waiver, and has not been through County screening for MH RTC. Last SUD/comprehensive assessment 4/28 recommended residential and last DA 4/2 also recommended residential.

Youth came into the ED 6/26 after being discharged from the shelter program they were in. Reportedly were boarding in a different hospital last week. Currently accepted at RTC but admit isn’t until 7/10. Needing an interim plan. County has permanent guardianship. Has been denied by most shelter programs. Does have waiver funding but no waiver CM yet.

Presented from local youth shelter. Unable to return to the shelter. Patient is in custody of Leech Lake Band of Ojibwe due to child welfare case that is currently open. No family available to care for the patient. Looking for any/all placements.

15-year-old female presenting to inpatient as a step up from PrairieCare Residential after experiencing increased mood dysregulation and suicidal ideation. This is her 4th hospitalization with PrairieCare and has been hospitalized at two other facilities. Guardians are currently hoping for a QRTP closer to home and not out of state.

Continue with established outpatient providers following RTC:
– Individual Therapy
– Medication Management
– CTSS
– Primary Care Provider

Youth came into our ED on 10/8 after altercation at placement and she can not return there. County is working on getting an updated DA completed while here for recommended care. Has CADI waiver open currently. Guardian is through Mille Lacs Band. This is her 8th time boarding in our ED. Referrals have been made to CABHH, MORA and some group homes.

Youth came to our ED on 9/4/25 from a group home after reportedly ingesting extra medications. It’s unclear how she got the medications, but her custodian does not want her returning to the home at this time. IP was initially recommended however she was declined from multiple IP units due to feeling there would be no benefit from IPMH and due to her acuity. She has since stabilized while waiting in the ED, and is now being recommended for discharge. This is this youth’s 7th time boarding in Fairview EDs. There is a tentative placement set up for the 22nd, looking for potential interim plans.

Pt is a 10 year-old female with historic diagnoses of anxiety, depression, ODD, specified neurodevelopmental disorder who presents today to address running away from home. Upon interview pt was observed to be easily distracted and appears to be guarded. Pt was resistant to the interview stating, “I don’t want to answer any questions.” After roughly 5 minutes pt refused the interview process leaving the room and forcefully pushing the door close with writer still in the room. Writer could hear pt yelling and laughing from the hallway “she’s locked in there” and “I locked her in there”.

Mother reports pt has been running away from home and putting herself in dangerous situation. Mother reports pt was sexually assaulted during one of her runaways. Mother reports pt’s motive for running away this time was to get on an airplane. Mother stated due to pt’s behaviors, pt has been hospitalized 3 times within one and a half weeks. Mother reports pt’s mental health is worsening. Mother reports pt struggles with authority figures and being held accountable. Mother states they use to have structure and routine in the home and is trying to incorporate it into the home again but when pt is held accountable, pt will threaten mother and think mother is “being mean.”

Youth came to our ED 4/17 from group home, who are not allowing her to return. Stearns County has custody currently and there is a CPS worker as well as ICWA advocate. No waiver currently. Had a SUD assessment that recommends residential treatment and she was agreeable to that, however no acceptance at programs yet and wait lists for those also. CPS is exploring shelter and kinship options. Was denied by Red Lake shelter and Ain Duh Yung.
