Child presented to the emergency department after getting physically aggressive with sister at home. Was brought via EMS to receive mental health evaluation. Pt was recommended discharge home after MHCA. Mother refused to discharge with child to return home.

This youth presented to the hospital after eloping from her foster residence. She had been on the run for over a month and was brought in by EMS after attempting to enter a youth shelter. She was admitted to the hospital for medical needs and determined to be appropriate for discharge. The foster residence is unable to take her back at this time due to needed home repairs and working on increasing staffing from 1:1 to 2:1.

Youth came into our ED on 7/20. Came straight from another hospital. Has previously been in hotel crisis respite however due to history of property destruction was not allowed to return to the hotel location and provider has not been able to identify another hotel that pt can go to. Is on waiver and team is working toward a single site home, however waiting for DHS to give provider their UMPI# before they can take pt, unclear how long that could be. Parents are refusing to take youth home in the interim.

Patient presented to ER following discharge from residential due to aggression to staff and eloping. No other placement locations as she is not able to return to previous foster placement. Patient with poor boundaries, impulsivity, and instigates other peers to engage in negative behavior. Appears lower functioning or potentially with ASD in which we will be completing psych testing.

Youth came to ED 6/17 after altercation with siblings at home. Adoptive Mom is not allowing her to return currently. Has boarded in our ER 2 other times, in April 2026 and May 2025, returned to Mom both times previously. Has DA and approval for RTC, primarily looking at RTC options. Trying to get further medical records to support RTC referrals from Children’s. No MNChoices/waiver currently.

Youth came to our ED 5/31 from a waiver placement that issued immediate suspension/termination. County has custody. Due to his age and some behavioral concerns, placement is difficult to locate. This is his 3rd time boarding in our ER in 3 years. They are currently searching for waiver placements crisis respite and CRS, and he is on the list for YSC, along with several other youth. He’s been on MITH wait list for years and remains on MCCP list.

Youth struggles to maintain appropriate boundaries with peers, engages in instigating behaviors and has a significantly strained relationship with her caregivers, which has continually contributed to disruptive attachments.

Youth came into our ED 5/8. Has been to our ED many times in the past, coming in almost weekly. Dad frequently brings him in as he feels overwhelmed by kiddos needs. Pt has behavioral concerns of elopement and some aggression. Has intellectual disability though it seems IQ testing has not been completed since he was 5, is mostly non-verbal. CPS is petitioning for custody. DD waiver is being opened, and crisis respite options are being explored. Reported that he is a “2 county kid” with both Ramsey and Hennepin involvement. Has a referral in for stabilization center however its unknown how long that list is or if he will be accepted.

Youth came into our ED 5/5. This is her 3rd time boarding with us in 3 weeks. She discharged to Dignity House and Aspen House the last times and is needing placement again as she can’t return to either. (Eloped from Dignity and got into an altercation at Aspen. ) Looking for waiver or treatment placement, but has been denied from many options.

Youth came to our ED 5/5 via parent after discovery of past sexual abuse toward sibling. Parent is refusing to take pt home. CPS doesn’t have jurisdiction to remove pt from home as his safety is not at risk. Current CMH worker is contracted, many steps would be needed if RTC were to be pursued/recommended. No waiver.
