Youth came to our ED 11/30 after an altercation in the car with adoptive parents and suicidal threats. Has been to Northwoods PRTF for year and Leo Hoffman PRTF for 6 months, which were not successful. Had been in the community for a couple of months before coming back to ED. Team is exploring possibility of return to Northwoods. Previously had a waiver through Morrison County but current dispute between Counties about where Case Management for that waiver will be- Hennepin County is digging into this, and plan to explore waiver placements once sorted.

Crisis stabilization services are mental health services, provided after crisis intervention, to help the member return his or her functioning to the level it was before the crisis.

Kiddo came to our ED 12/23, came from North Memorial ED after incident with Grandma at home, brought to us from County Case Manager. Grandma has custody. Has history of physical aggression, primarily directed at family. Has been calm and cooperative in our ED. CM is currently exploring parent referred shelter programs but all thus far have denied due to age/behavior. OP therapist is recommending RTC for long term plan but needing interim plan as well, Grandma reports it’s not safe to return to her.

Kiddo came to the ED 9/19 after aggression at his group home. Boarded at our hospital summer of 2023 for a few months. Team does not want him to return to his group home and is seeking crisis respite while they work on finding a new group home. Has DD waiver.

Came to our ED on 11/28 following and altercation at home with Grandma who is custodian, that led her to eloping 11/27 and then was picked up by police. Grandma is refusing to let her return. CPS is getting involved as family situation is complicated and not ideal. Previously boarded in our ED 4/24-5/29. Has previously been denied by most residential programs. Needing an updated DA- which County and our team are trying to work on, but due to previous residential denials are not confident this will help with placement. Has CMH worker.

Came to our ED on 11/25 after altercation at home with Aunt, who is current custodian. Aunt has refused to take her back home and is refusing any involvement in her care currently. CPS is getting involved. Has CMH worker as well. Nexus YCT has been involved ongoing and is trying to work on referrals for residential, but needs ROIs first. Has interview with residential provider who previously accepted today 12/3.

Patient with autism and agitation sent from his new group home awaiting placement options. pt’s DD CM Deqa (612-472-2572) to gather more information regarding the Exception Bed Referral. Deqa shared that she was told that she can either refer for the Exception Bed or MFOC but not both. She is moving forward with the MFOC referral, as it would be a long term placement which would be better for the pt in the long run. She has made it known that the referral to MFOC is urgent. If the MFOC referral is denied, Deqa plans to either push on DHS as they have had an opening for 1 year and have not filled it, and/or pursue the Exception Bed. Deqa said she submitted for the Minnesota State- Operated Community Services (MSOCS) referral on 10/15. DHS Complex Transitions Team referral made on 10/15/24.

Kiddo came to our ED 9/25, brought in after being discharged from NW Passages, where she was for 3 months, due to elopement and being found by police. Aunt brought her to our ED. Has CMH through Anoka County who are working toward placement, concerns about SEY history and elopement.

Came to our ED 9/22 from home due to aggressive threats. Previously was IP with our hospital 9/3-9/12. Mom does not feel she can take kiddo home at this time. Is in process of obtaining Childrens Mental Health CM and CADI waiver, but has neither as of now (did request these to be expedited.) Mom is also working on referrals to Residentials (has private insurance) but has not been placed on any lists yet.

Came to ED after altercation with mother. Ongoing family conflict between Mother and Daughter. CPS has just began getting involved on case. Currently mother is telling our team we must admit to IP and has not been willing to sign any ROIs to proceed with anything as of yet. Unable to proceed with any placements, waiting for further direction from CPS.

This pt discharged from our ED on 8/6 after being here since 7/22, went to a Beacon crisis home for approx 24 hours and returned to the ED again on 8/7, after aggressive behavior. Previous number was MHF-M12-4418. Also boarded in our ED in April 2024 for 20 days, and at different hospital in June. Hennepin County has temp custody and he is on DD waiver. Currently looking at a CRS home that requires exception bed approval through DHS, Grafton, remains on the MCCP crisis pool list, and judge is making determination if returning to Mom may be an option. Has been to Zane crisis, Beacon crisis, VOA Bar None, and done PHP with FV.
