Youth came to our ER on 10/9. He had eloped from his home last week, police found him and when brought home he physically aggressed toward his mother, so police brought him to JDC, they reported they could not keep him because Mom was not pressing charges so he was brought to the Bridge shelter, where he was discharged from for exposing himself to others and was then brought to the ER. Mom is refusing to pick him up. He has autism and selective mutism. There is a recommendation for PRTF and RTC from June however the County has not yet screened via JST. There is not a current waiver, though waiver placements are starting to be explored to rush that assessment if identified. Has previously been denied by Hoffman Center and Grafton.

Admitted due to suicidal and homicidal ideation. Patient struggles with explosive outbursts when she does not get her way. Patient has a hx of calling CPS frequently alleging abuse against mom. Diabetes was managed with an insulin pump until recently, when the pump was removed for inpatient admission and she was transitioned to insulin injections. She reports that the pump was helpful in regulating her diabetes. She reports daily suicidal ideation, describing it as a persistent struggle, with plans to smother or strangle herself. She also experiences homicidal ideations, particularly directed toward her mother and sister.

Youth came to the ED 8/16 after altercation at home. Mom is refusing to allow him to return. County is seeking shelter options for immediate plan, limited options with his age. No current DA for treatment recommendation and limited resources from the County. Parent had submitted MNchoices application last December but previous CM never submitted it, has been re-submitted now and requesting expedition due to mistake on CM part. There are no kin/family placement options currently.

Youth came into the ED 7/2, from home, after being discharged from Fairview’s IPMH program on 7/1. Mom is refusing to allow her to return home. IP unit set pt up with attachment-based therapy to start 7/2, day treatment to start 7/9, psychiatry appt 7/9, and a referral for CTSS when available. Parents are declining to take back with those services. County offered CIBS services again as well, parents refused. Parents state they will only take home with 24/7 in home care which isn’t an option. Parents also state they will only accept RTC or a permanent group home (CRS) There is no current LOC recommendation for RTC, and pt just got onto the wait list for MNchoices assessment yesterday for the 1st time, so neither are options currently. Parents are not allowing referrals to parent referred shelters as those are not permanent solutions. No placements being explored currently because parents aren’t agreeable to current options., and no CPS involvement for County paid shelters.

DA currently recommends level 5. Team is working on referral to Leo Hoffman today.

Youth came to our ED on 3/24 from home after an altercation with Mom. He has a history of physical aggression against Mom. He is commercial insurance, so there is currently no County involvement. Mom would like him to get RTC however there isn’t a DA with that recommendation currently. Mom is not currently agreeable to shelters.
