Was hospitalized in at Prairie Care Psychiatric Hospital discharged May 30th- June 15th for significant self-harm by cutting, recommendations were residential, was accepted to Newport, Prairie Care RTC, and Rogers; however, due to bed availability was discharged home and started Prairie Care PHP on June 16/17th. Wound became infected on leg, he was hospitalized and treated for Sepsis, wound vac was placed. He was discharged home with recommendations of residential treatment once wounds healed. He was unable to go back to PHP or Residential (there was a bed opening at one of the placements) he was unable to return to PHP or be admitted to Residential due to wound vac/wound care needs. Within 24 hours of discharge, he was back in the hospital due to self-inflicted wounds on his leg caused by a blade of a box cutter requiring medical intervention and hospitalization. He has been in the hospital since his wounds are healing, there is no longer a wound vac, 9 or the 10 wounds have been sutured and one on his leg requires wound packing 2x per day.

Patient is well known to our ER/Hospital. She previously boarded in the ER 3/12/25-5/9/25 when she was transitioned to the Adolescent inpatient unit and boarded until 10/2/25. Patient moved into a group home built for her.
Patient returned to our ER on 6/28/26 and the group home suspended services effective 6/29/26 due to the level of aggression patient has displayed and injuries to staff. Sherburne County and patient’s guardian (adoptive mom) are not appealing.

9 year old female recommended for inpatient care due to verbalizing suicide plan. County involved. Outpatient services minimally utilized. MCO has been in the home several times and offered recommendations to family. Parents active and present, but unusre if able to set boundaries or have skills to meet needs. Other kids in home. MCO eval recommended placement and continue in ED until placement located. Avel consult and suggested could go home with safety plan if parents agreeable, parents not agreeable. Physical acting out example pounding on window, reported destroying things at home.

Several referrals with denials due to the ingesting. looking at Group homes. takes responsibility for her actions. Parental rights terminated. Barriers are not being able to keep her safe (ingesting). medical issue with pancreas – very specific diet, ex. places don’t have medical staff on site. Turns 18 in May

Patient is presented to the ED by EMS from an RTC facility. Patient assaulted another resident and staff members, and attempted to elope from the facility. Patient is under guardianship of Martin County, and is considered a vulnerable adult. Patient has a significant history of abuse, and suicidal ideation. Patient is supposed to follow a low sugar and fat diet due to pancreatitis.
