Update 2/19/26 

– Child and Adolescent Behavioral Health Hospital (referral submitted, under review, current waitlist is weeks to months) 
 PRTF/Level 6 (Approved by AFMC for PRTF) 
– Nexus East Bethel (referral submitted, able to be added to waitlist when off 1:1)
– Northwood (reviewing, waitlist 3-4 months) 
– Grafton (declined due to inability to support level of care) 
– Leo Hoffman Center (referred twice, declined twice) 

QRTP/ Level 5: 
– Return to Nexus Gerard (declined to return. Discharge effective 9/12)

Update – 2026-02-05 04:00:10

Continues to board in the ED. DHS approved ICPC and plan was to discharge to family in WI 2/4, however family now said they can not take youth until 2/16. Continuing to look for a short term option in the interim.

Update – 2026-01-23 02:07:57

Patient was discharged home 1/22/26 and returned to the ER several hours later after reported drinking cleaning products.

Update – 2026-01-22 05:26:58

We were told 1/20 that waiver options, CRS and crisis respite, are all being denied by the County. And pt is going to be unable to return to the Wingspan home, that bed went to someone else. Unsure what potential options there are for this youth at this point.

Still awaiting confirmed admission date for facilities. Patient has been stable with no aggressive behaviors or elopement attempts

County screening meeting approved 35 day assessment verses residential or crisis bed. Anticipated 4-5 month wait. Looking at discharging home with community supports while awaiting admission for 35 day assessment
