After discussion with Anthony Louis CD treatment they are willing to take another chance on pt, and he will be discharging today 12/4.

 Referrals: 
 -Gerard Academy: referral being reviewed 
 -Nexus East Bethel: referral being reviewed 
 -PrairieCare Residential: declined due to acuity 
 -Avanti: declined due to acuity 
 -North Homes: tentatively accepted pending county funding 
 -Bar None Haven: Anoka County has said they will not fund 
 Anoka County initially declined to complete QRTP screening although is now reconsidering. They are currently recommending PRTF.

Update 12/3/2025: 

 RTC referrals have been made to – 
 North Homes: He is accepted pending approval of county funding 
 Gerard Academy: Referral is being reviewed 
 PrairieCare Residential: Declined due to presence of Intellectual Disability

Update 11/13/25- Pending admission to CABHH, accepted on waitlist, pending if he needs his own unit there or can be on general unit, waitlist unknown.

Update – 2025-11-10 06:25:45

Hoffman has declined him as well. Has been denied by all PRTFs, QRTPs, and SUD residential treatments in the State of MN. CABHH has also denied 2x, MORA has denied. DOC facilities and out of state placements not options due to commitment. Team is looking at waiver placement options, no leads thus far.
