Guardians requesting PRTF at Leo Hoffman (in review, waitlist a month). County also sent referrals to Village Ranch (accepted, admission in August) and Northwood PRTF (on waitlist).

Update – 2025-07-28 02:47:17

Continue to look for appropriate placement for this youth, in and out of state. Looking at all options, RTC, group home, waiver, anywhere that may accept him. MNchoices was completed while in the ED. JST recommended RTC. Has continued to be behaviorally appropriate for 1.5 month in the ED, no behavioral issues since being here.

Stabilizing over the weekend. Hoping for admission to Avanti next Tuesday 7/22 or Gerard- Interview with Gerard on 7/18.

-Gerard Academy (Referral sent, reviewing)

Update 6/9/2025: This youth discharged from PrairieCare Inpatient Hospital with a direct admission to Gerard Academy RTC.
