Update 6/6/2025: This youth discharged from PrairieCare Inpatient Hospital on 6/6/2025 to community based services while awaiting admission to Nexus East Bethel.

Another connection was made with CMH and they are reportedly continuing to deny until after placement is found. No current CASII reported- CPS is seeing if that can be done in while in the ED. Working on getting MNchoices expedited to open waiver placement possibilities. CPS submitting foster care referral. County took custodianship 6/4.

CM is trying to work to get a single site development made for pt, and due to the long nature of this process, wants the same provider to provide hotel crisis respite in the interim, but needing many approvals for all of those pieces. Is also exploring other waiver placements, MH placements, shelters, out of state residentials, any where that may accept this kiddo.

Client will be having an admission at Leo Hoffmann Center on Monday 6/9/25

Accepted to Genesis Crisis Respite and met with a foster family. Has a discharge date for 6/12/25.
