Update 3/12/25: 

 Establish Residential Treatment; 
 -Avanti Center for Girls, insurance not accepted 
 -Nexus Gerard, pt declined due to admission not being available prior to pt’s 18th birthday 
 -North Homes, pt declined due to assessment of needing higher level of care 
 -Newport Academy, pt initially declined due to assessment of needing higher level of care. Requested re-review, pt was accepted to the Orange County CA location. Guardian declined 
 -PrairieCare Residential, pt declined due to eating disorder concerns 
 -Bar None Haven, not an option due to insurance not being accepted 
 Consider out of state RTC – referral made to Acadia Healthcare. Guardian now declining out of state options. 
 Children’s Mental Health Case Management – guardian declined referral at this time

Update 3/12/25: Hennepin County is working to schedule JST meeting. RTC referrals are being reviewed at Bar None Omegon Ascend, Gerard Academy, North Homes, and Nexus Mille Lacs. Accepted to PrairieCare Residential pending county funding.

Update – 2025-03-06 09:15:34

Update 3/6/25: Hennepin County approved funding for QRTP. Patient is accepted at PrairieCare Residential with an estimated 2-3 week wait list until an opening.

IRTS can not accept him without being 18. SUD Assessment was completed and recommended outpatient, as this is what pt is agreeable to at this time. Has not been approved for CADI placements and MH placements aren’t an option currently due to age.

Update – 2025-02-27 02:04:31

Patient is currently admitted at another medical facility due to ongoing medical concerns. Patient is planned to return once discharged from that facility. Patient is still scheduled for admission at Nexus East Bethel on 3/3/25.
