Patient had a meet/greet/observation with a group home that is interested in having patient move in. Patient moved to the Pediatric unit to continue boarding 

Update 3/17/25: Patient admitted to PrairieCare Residential on 3/17/25

Discharged to uncle 3/14

After a meeting with GH and Custodian at the hospital, pt was agreeable to return and go to school, so returned to previous GH.

Discharged home with Mom and some services set up 3/14

Discharged 3/7/25 with case manager.

Some IRTS referrals have been made with anticipation of turning 18 in a couple weeks. Continuing to look for adult waiver GH placements.

CPS has been able to get the CMH and Waiver process within the County moving faster, so both are in progress. CPS is connecting with kin placement possibilities and is working with family for possible VPA.

Has potential placement with crisis respite on 3/18.
