1/20/25: Patient has been accepted to Nexus East Bethel with a tentative admission date of 2/19/25.
Patient is also on the wait list at Bar None Haven and is third on the wait list.

Update – 2025-01-24 01:31:45

East Bethel unable to accommodate in the near future. Now exploring alternative placements and referral was made to VOA Haven.

Update – 2025-01-17 04:05:17

Patient discharged on 1/17/25 into the care of their legal guardian. Patient will be utilizing wraparound services with Hennepin County.

Hotel crisis respite provider that has been working to set up services for this kiddo for almost 3 months, notified team on 1/9/25 that they would no longer be willing to provide placement for him, so after almost 4 months of boarding in our ER we are back to square one, and re-starting placement provider search. Nexus team is reaching out to Zane Care and Living Hope hotel crisis respites, along with many other providers. Primary difficulty is finding a provider that can provide patient 3:1 staffing, and have a van with modifications for safety to provide transportation to and from school, County has already agreed to cover the cost of these vehicle modifications and agreed to 3:1 staff.

-Leo Hoffman PRTF, referral made, declined for treatment due to FSIQ, re-referral being made with updated psychological testing

-Northwest Passage, referral made, reviewing, 4-6 week wait if accepted

Establish CMHCM;

1. Referral made to Hennepin County on 11/26/24, and assigned to Song Vang (song.vang@hennepin.us) for intake

2. Song Vang submitted referral for the patient to the MN SMRT Team for Waivered Services

3. Reassigned to Michelle Lee (612-990-6213, Michelle.Lee@Hennepin.Us) at Hennepin County for Wraparound Care; intake meeting scheduled with provider and family on 1/21/25 at 0900 via TH

Shelter option mentioned in last update fell through due to confusion from shelter about County paid vs family referral. Interviewed with a mental health group home 1/15 and went well. Plan to get update from them by 1/16 if pt was accepted.
