County has opted to decline placement due to concerns regarding the facility not following license requirements and safety concerns for the patient. Restarting the placement search.

Patient remains boarding in the ER. Patient has been accepted for admission to group home- Actively Living Life. There have been delays in setting a date for admission due to the group home needing to adjust their current license to add an additional bed.

      referral made to Hennepin County on 11/26/24, assigned to Song
      Vang, met with patient and guardian on 12/23

      -Consider Forensic Evaluation; 
      -Continue Psychiatry private practice
      -Establish
      Occupational Therapy; Minnesota Autism Center in Minnetonka,
      referral made on 12/2/2 or Sora Therapy in Chanhassen,

PRTF or RTC recommended, due to no availability facility and extended hospitalization, discharging home with wraparound services. 

 Plan for Wraparound Services: 
-Establish Individual Therapy with at Lorenz Clinic-Prior Lake (12/30/24 at 1400) 
-Attend Care Transition Appointment with PrairieCare Hospital, Telehealth (12/31/24 at 1500) 
-Continue Medication management at Shifa Metro Clinic (1/8/25 at 1440, in person) 
-Continue PCP at Park Nicollet Burnsville (1/2/25 at 0945)
-Continue Dakota County CMHCM 
–Continue with Dakota County Crisis Services 
-Establish Washburn Crisis Stabilization/WARM Program (Referral sent, parents can call Washburn intake to confirm info and patient will be re-added to waitlist. List is currently 2-6 weeks) 
-Continue Dakota County Child Protection as required by the county 

 RTC/PRTF Referrals pending- 
-Bar None Haven (referral made, waitlist about 6 weeks) 
-Nexus East Bethel PRTF (referral made, waitlist is several months out) 

 RTC/PRTF Referrals Considered- 
-PCR (referral made, declined due to acuity) 
-Avanti Center for Girls (referral made, declined due to acuity) 
-Gerard (parent declines referral at this time due to distance from the home) 
-North Homes (parent declines referral at this time due to distance from the home)

-Grafton PRTF (referral not appropriate, facility only accepting individuals with ASD, ID, DD- pt does not have)

Rates for the hotel provider have been approved, van mod was approved, just waiting for van modification to be complete and then pt will be able to discharge. Set tentatively for 1/3 or 1/6.

Has been accepted to RTC but their timeline is at least 5 weeks out. Needing an interim plan. Not open to waiver so unable to do hotel crisis or other crisis respites at this time.
