9/26/24- 

 From an acute psychiatric standpoint, youth is safety to discharge form inpatient care and receive services in the community prior to admission to PRTF. Last (and only) incident of externalizing behaviors that required physical staff intervention was 7/28/24. Patient has made significant progress on treatment goals during hospitalization. 
 PRTF-(county determination/recommendation – DHS funding approved) 
 -Nexus East Bethel (referral submitted; in review) 
 -Grafton (not an option; patient does not meet admission criteria) 
 -Northwood (referral submitted; two year waitlist) 
 – Acadia (referral for out of state consideration submitted) 
 Cannot refer to Leo A Hoffman PRTF as they only accept males at this time. 
 Out of State: 
 -Rolling Hills Hospital (patient clinically accepted and added to waitlist) 
 -Lakeland Behavioral Health (patient clinically accepted and added to waitlist) 
 -Resource Treatment Center (reviewing) 
 RTC: (county declined funding for level 5 QRTP, recommended PRTF, family may be pursing 3rd Path now for RTC)
 -Avanti (declined) 
 -Gerard (referral sent, unable to move forward due to lack of funding) 
 -North Homes (referral sent, unable to move forward due to lack of funding)

Patient has been accepted to Chileda in Wisconsin with anticipated admission date of the end of October or beginning of November.

Patient’s county case manager continues to look for placement and is working to get CADI reinstated along with working with probation on possible options.
