Establish MN Choices Assessment; 
-through Hennepin County, SW spoke to provider who will submit referral and consult with team, will follow-up by 10/23 
 RTC: 
-Avanti Center for Girls; patient unable to attend due to funding source, could potentially attend if funding was supported by Mower County 
 Continue with established outpatient providers: 
-School-based therapy 
-PCP: Mayo Clinic Austin 
-MM: Mayo Clinic Albert Lea 
-Child Welfare Case Management: Mower County 
-PO: Mower County 
 Estimated length of stay: 
Seven to ten treatment days, or until patient stabilizes and aftercare is established.

      24-hour supervision.

10/14/2024: Funding was approved through the county for QRTP. Patient accepted to Gerard RTC with available admission 10/15. Thank you Gerard!

Update – 2024-10-10 03:00:55

continues to be hospitalized on our inpatient mental health unit. He is stabilized and ready for discharge, but we have not been able to secure placement. Dragonfly is considering him, but this has been in process since August with no concrete plan or admission date to their program. Referrals have been sent to Genesis Crisis Respite-pending, Dungarvin (no open beds), and LSS (referral still pending). St. Louis County is working on a referral to Tomorrow’s Children-a residential tx program in WI.
