Admit date to RTC was pushed back for a 3rd time to an unknown timeline. If not admitted before turning 18 in a month, would not be able to go any longer. Looking for placement options that may accept. Has been denied by many shelter programs and all RTCs besides the one mentioned above. Has CADI waiver eligibility and adult and childrens homes are options, one month from 18.

Boarding on the EmPath unit. Tribe has “no updates”, looking for relative placement. St. Anthony declined due to no CD concerns. Has been behaviorally appropriate here without self harm or acute psychiatric/medical needs.

County placement screening team approved out of home placement 6/11 so County is now looking at CRS and crisis respite via waiver, Group Homes, RTCs, and PRTFs, as well as shelters and foster cares. Pt has never been in out of home placement previously. Has DD waiver with IQ of 82.

6/12- accepted to Hoffman PRTF, waitlist 1-3 months. Seeking interim care, parent doesn’t feel safe having youth home. Referral to CM done and intake completed. MN Choices referral done, pending intake.
