Pending referrals with: Grafton- interview this morning 9/24, Luther Hall- ND, some waiver crisis respites and CRS. MNchoices took place yesterday 9/23.

Plan is for pt to discharge today 9/24 to a respite care home for a couple of weeks before easing back into Grandma and Grandpa’s home.

Focused living continues to visit her weekly and are hoping to close on a home in the near future. Patient is doing very well with outbursts around 1-2 times per week which is a significant improvement.

Patient has been denied from several locations due to facilities not feeling that they can meet her needs. This is a young girl that really needs 1:1 attention and would strive in a smaller environment with plenty of positive attention and nurturing. County has not yet looked out of state as they feel the court will not approve it as she is split up with a sibling and has not had contact with this sibling for quite some time. The patient really just wants someone to give her a chance and offer her the positive attention she craves.

Acadia, Kidlink/UHS, Prairie Care Residential, Gerard, Heartland Girls Ranch, Little Sands, Prairie Lakes Youth Program, Red River Valley Juvenile Center

Update 9/16/2026: Declined at North Homes and Northwood.

Referrals still pending at Village Ranch and Heartland Girls Ranch. 

Talking with guardian about possibility of out of state RTC referrals.

NW Passages discharge recommendation is residential. Northwoods PRTF and Chileda have extensive wait lists. Referral is in for Grafton. Gerard denied. MNchoices request continues to pend. Exploring in and out of state options, welcome to all suggestions.
