Patient was accepted by Focused Living to create an individual group home. Currently in process.

This youth remains in the ER. Has been here for 22 days and unfortunately there has not been forward progression to the ongoing provider getting their UMPI#, connecting with DHS on this has been difficult. And an interim plan has not yet been identified, though crisis respites are being explored, there are limited openings for youth. DA is currently being updated. It’s mentioned that RTCs were previously explored and many denied due to autism diagnosis, and Grafton previously denied due to sexually inappropriate behaviors.

DC to group home on 7/21. Since DC , he has run away and was picked up by police who brought him to detention where he remains.

was discharged from hospital to SERCC 7/17. Got another infection of E. Coli in his wound, SERCC had to discharge as his wound cares were too high and they could not provide the level of supervision to ensure that he is not manipulating the wound. Youth was discharged 7/24/26 with recommendations to go back to the ED because of concerns around how the wound was healing. Parent refused and brought him home. No services are set up in the home, although we have connected with the Complex Transitions Team, and CBS services through DHS. We have home health agency willing to provide nighttime supervision, but they are working to hire staff currently.

This youth has a potential adult waiver home placement and a potential YSC bed as a back up plan for discharge next week.

Had a neuropsych eval last week and has made recent changes to meds. Many behaviors are due to his cognitive delay, not dx with a dev. disorder. IQ is 72. Concerned with safety.

Patient transitioned to the adolescent inpatient unit on 7/7. She will continue to board while the county works with a potential provider to develop a new moratorium.
