This pt was discharged to a Beacon Crisis Home for 24 hours and returned to our ED after some aggressive behaviors.

8/12/24: Patient discharged from inpatient on 8/12/24 with direct admission to Bar None Omegon Ascend RTC.

8/15/24 Update- This pt has moved to IP unit to better manage behaviors while she boards in the hospital. My team will no longer follow.

Patient discharged to a crisis respite group home on 8/9/24

Patient had meet and greet with REM group home 08/02/24. REM wanted additional notes from previous group home which county worker received on 8/9/24 and forwarded to REM.

Weekly supervised visits with mother, placement interview completed with Grafton PRTF on 8/1.

Patient remains hospitalized on the inpatient adolescent unit at St. Cloud Hosptial.

Patient accepted to a DIVINE home with admission date pending
