Discharge Plan:
From an acute psychiatric standpoint, youth is safety to discharge form inpatient care and receive services in the community prior to admission to PRTF. Last (and only) incident of externalizing behaviors that required physical staff intervention was 7/28/24. Patient has made significant progress on treatment goals during hospitalization.
PRTF-(county determination/recommendation – DHS approved)
-Nexus East Bethel (referral submitted; would re-consider in several months until milieu stability at East Bethel)
-Grafton (not an option; patient does not meet admission criteria)
-Northwood (referral submitted; two year waitlist)
– Acadia (referral for out of state consideration submitted)
RTC: (county declined funding for level 5 QRTP, recommended PRTF)

9/13/24: Patient discharged from inpatient. Patient discharged home with PHP and outpatient services due to no available stepdown for PRTF – patient was declined to Grafton PRTF due to their current milieu and declined to Nexus East Bethel due to her age.
