Was given a CASII score of 6, but also recommended a CADI group home. Need clarification on this. Nexus YCT is involved.

DC scheduled 6.27.24.
Discharge is indicated as he has met the maximum benefit that Inpatient hospitalization can provide. He does not have acute psychiatric safety concerns. He no longer meets criteria for inpatient hospitalization. Parent does not feel safe taking pt home, asks for shelter options.

-Bridge for Youth Resilience House (no beds available 6/26)
-Hope House (no beds available 6/26)
-Brooklyn Park Avenues (cannot refer, age 16+)
-LSS Safe House (cannot refer, age 16+)
-Tubman Safe Journeys (cannot refer, age 16+)
-Bridge for Youth Marlene’s Place (cannot refer, age 16+)

-Continue Prescriber/Medication Management: Nystrom and Associates in Eden Prairie
-Continue CMHCM: Sara Halvorson with Hennepin County
-Establish Crisis Support with Nexus Family Response and Stabilization Services

RSI screened today to reconsider accepting her back into their program. No other options for placement have been identified at this time. The county has approved a nationwide search placement.

Update 6/26/24: 

 -North Homes: approved to transition to shelter program while awaiting opening for 35 evaluation. Waiting on county funding approval. 
 -Nexus Mille Lacs: referral under review, wait list approximately 4 months 
-Gerard: referral under review, wait list into the Fall
