Weekly supervised visits with mother, placement interview completed with Grafton PRTF on 8/1.

Patient remains hospitalized on the inpatient adolescent unit at St. Cloud Hosptial.

Patient accepted to a DIVINE home with admission date pending

Patient transitioned to the SCH Adolescent MH unit on 7/29/24

Nexus YCT working to find placement in collaboration with county

Update 7/25/24:
Patient discharged from inpatient with direct admission to Nexus Mille Lacs RTC

Youth accepted to Gerard and is pending an open bed. Looking at CADI home for interim placement but waiting on cadi waiver.
