Two homes are available through Wavier- however Polk County doesn’t have a bed to transfer to admit to the two current options. Working with other counties to see if they will transfer the bed.

Looking at potential discharge home with mom the week of 5/11

Patient will discharge home on 5/12 while awaiting residential placement.

Update 5/7/26: Accepted to Stonecrest Living Group Home. Guardian is reviewing the program.

Has been accepted for waiver crisis respite home, awaiting other pt to move to their next setting before she can move in, will hopefully be next week.

Guardianship hearing took place 4/30 however judge has not signed paperwork, so pt remains own guardian until that’s processed. Could take up to 90 days but more likely will be 2 weeks. Continuing to look for adult crisis placements, or CRS, no options thus far.
