4/29/26: County has declined this youth for crisis services through his CADI waiver therefore was unable to admit to Genesis CRS. Continuing to navigate appropriate setting for this youth as he continues to board at inpatient.

04/29/2026: Accepted to Leo Hoffman PRTF with planned admission for 5/4/26

Mom involved but unable to go back home at this point.

Working toward a QRTP approval, no “funding” for any placements yet.

4/26/26- declined to all QRTP/PRTF, waiting to hear back from Nexus East Bethel, planning to discharge to wraparound services on 4/27

Update 4/23/26: Can admit to Genesis CRS on 4/28/26, pending CADI supervisor approval for the daily rate cost.

Patient has been accepted to Northwest Passage. On wait list, admission end of May. We are still seeking respite care while awaiting residential placement.

Low IQ score is a barrier to placement
