Updated DA is recommending therapeutic foster home, needing screening to take place to authorize that level of care. Looking at waiver crisis respite and CRS options also. Parents refusing parent referred shelter due to not wanting to pick pt up should she dysregulate.

 -Gerard Academy (Referral submitted 4/9, accepted 5/6, estimated waitlist 1-2 weeks dependent upon approval of county funding) 
 -North Homes (Referral submitted 4/9, unable to make admission determination until county funding is approved) 
 -Bar None Omegon Ascend (Referral submitted 4/9, accepted 4/29, estimated waitlist 6-8 weeks) 
Establish CMHCM: Ramsey County – Intake CM Roberta(Referral submitted 3/31 via email, re-sent 4/1 via fax & 4/9 via fax; re-faxed 4/16 to corrected fax number, confirmed received 4/17; re-faxed 5/1 to supervisor. Intake mtg completed 5/6 with LG, awaiting return calls) **JST meeting will need to occur/county funding approved prior to admission to RTC** 
 Bridging Plan (not currently appropriate, but pending symptom improvement):
 Consider PHP at PrairieCare Brooklyn Park/MOB (SW to re-refer as clinically appropriate) 
 Continue with established outpatient providers: 
 -Therapist: Fuller Living & Associates (Weekly appointments, Wednesdays at 1600) 
 -Psychiatrist: Introspect Mental Health 
 -PCP

Update – 2025-05-16 03:18:04

Hennepin has custody currently, and during hearing judge ordered no shelter placement. Currently working on Foster Care and have not had luck there thus far. Team completed DA 5.14 that recommended PHP LOC. Has been calm and cooperative in our ED for 3 weeks, no behavioral codes/concerns. While with aunt there was only verbal conflict reported.

Continuing to search for waiver placement CRS/crisis respite/hotel crisis. Team is getting clarification with DHS how many days hotel crisis can be approved for, unsure if this is an option. Team doesn’t feel shelter can meet her needs. No recommendation for MH treatment placement currently.

Update – 2025-05-16 02:58:03

She has been at times agreeable to SUD residential treatment, and not at other times since being with us. During times of agreement referrals were made to FV Maplewood, Wings and Oshki Manidoo programs who all denied. Referral pending with Anthony Louis. CFR switched from Stearns County to Hennepin County 5/12/25. Hennepin now working on their process approval for CMH and MH GH placements. Continuing to search for kinship options. Had Nexus YCT involvement but unclear if they can remain open with the transfer to Hennepin Co.
