Leo Hoffmann Center’s clinical team has a virtual interview scheduled with the client and his support team Thursday May 15th to see if our programming will be a fit!

Update 4/30/2025: 

 This youth discharged from PC Inpatient Hospital with outpatient supports in place. 
 The PrairieCare treatment team recommends a residential treatment (RTC) level of care. 
 Referrals have been submitted to: 
 -Nexus East Bethel: declined 
 -Northwoods: not a viable option due to wait list being 2+ years 
 -Grafton: does not meet admission criteria 
 -Leo Hoffman Center: does not meet admission criteria 
-Heartland Girls Ranch: a referral has been submitted 
A chemical dependency evaluation was completed while at PrairieCare, the recommendation is for dual diagnosis residential treatment, guardian does not want referrals sent at this time due to pursing mental health RTC. 
 Establish partial hospitalization programming (PHP) to bridge to RTC, guardian does not want to pursue at this time.

She does not agree to residential treatment at this point. Recently had a MH DA recommended a PHP level of care. Looking at shelters and group homes, lots of denials due to substance abuse.
