Another Update for 5/1 🙂 

Discharge Plan: 
Residential care-
PRTF:
(eligibility form sent to AFMC – approved on 4/11) 
-Nexus East Bethel (declined 4/16 as they feel pt should attempt a least restrictive setting) 
 -Northwood (parent revoked consent on 4/10 due to distance) 
 -Leo Hoffman (not viable option due to only male patients) 
 -Grafton (not viable option due to needing ASD, ID, or DD 
 Level 6 RTC: 
-Bar None Haven (Parent consented 5/1, referral in progress)
 Level 5 RTC: 
-Avanti Center for Girls (mom’s first preference- declined due to increasing aggression and poor boundaries, recommending PRTF level of care) 
 -Gerard (referral made, consent revoked on 4/25) 
 Consider community based care settings such as group home or other residential living options (county not supporting at this time)

5/1/25 

Establish Residential Treatment; 
-Newport Academy, declined due to needing higher level of care, requested re-review, 3/5/25 pt accepted to Orange County CA, guardian declined referral due to distance and financial reasons and is unwilling to complete hardship application (Newport said hardship application would cover remaining costs and they could provide airline ticket for guardian and patient) 

Consider out of state RTC due to very narrow network of options with insurance –
– Acadia Healthcare, pt reviewed to McCallum Place, guardian declined referral due to distance 
– Rogers Behavioral Health, guardian declined referral at this time 
 RTC Declines: 
-Avanti Center for Girls, insurance not accepted 
-Nexus Gerard, declined due to admission not being available prior to birthday, as well as age 
-North Homes, declined due to needing higher level of care 
-PrairieCare Residential, declined due to eating disorder concerns 
-Bar None Haven, not an option due to insurance not being accepted 

 Children’s Mental Health Case Management – guardian declined referral at this time 

 *Insurance Highmark BCBS is declining further Inpatient hospitalization though youth is acutely needing inpatient care

The property purchase made by Anthurium in Stearns County fell through due to not having capacity to support a second out of county child moratorium. Will be starting from scratch with the goal of purchasing a property in Sherburne County.
