Sherburne County has obtained approval for creating a group home for patient. Met with Anthurium Consulting Group and they have confirmed interest in working with the patient. A property was purchased and now are working with DHS and county licensors.

This patient continues to be ready for discharge and needing immediate placement, however was transferred up to our inpatient unit for a better environment for her to wait in for her safety. MNchoices meeting was completed however continuing to finalize paperwork which has been difficult due to lack of communication with Custodian. County has said there are likely not waiver placements that would accept pt anyway due to level of care. DA was updated and recommended PRTF. Working toward in and out of state PRTF options. Nexus YCT was able to get their intake completed and is starting to assist with referrals.

Patient has been able to move out of the BICU with no further attempts to harm himself or others and has been free from any behavioral/emotional dysregulation for 2 weeks now. He has been accepted to Bar None with anticipated admission date in mid May.

Referral for waiver services is on Goodhue County workers desk, but doesn’t have MA, so it will be a bit to get things sorted. 180 degrees reviewing.
