Hennepin County waiver CM has been assigned. County plans to look at Crisis respite. CMH is contracted so unable to approve RTC at this time. Also does not have DA recommending RTC.

Has reportedly been accepted at a RTC, waiting for further information on timeline for this.

Previous GH provider is working on temp placement for pt while working to find another long term GH. Variance approved by DHS for that temp setting and hoping for discharge next week.

Have a potential CRS bed identified, pre-intake taking place 12/13, hopeful for placement next week.

Kiddo was accepted at Aspen House and plan is to discharge today, 12/5.

Patient moved to Adol MHU to allow for age-appropriate groups and interaction.

Only referral made thus far is CABHH, made by Nexus EB and no further information on status of that referral at this time. Is just getting CMH CM assigned this week. Some discussion taking place about return to aunt with services, MNchoices happening 12/6.

Currently working on getting hotel crisis set up with 3:1 staffing through a provider who will also be able to provide a long term single site development placement via a moratorium bed. Needing rates approved, as well as modifications to a vehicle and approval for payment of that, before being able to proceed.
