11/07/2024- Patient discharged to home with outpatient supports in place while awaiting admission to Northwood Children’s Services residential.

Update – 2024-11-06 07:29:08

Probation has determined they will not support placement at a juvenile detention facility and feel it is child protections job to assist with placement. Probation feels they should have no involvement in this case. Sanford Thief River declined to review as they do not feel they can keep patient safe.

Update – 2024-11-06 04:19:29

Referral was made for Mille Lacs Band psychiatrist. Referral for Scott County Juvenile Alternative Facility has been made. Request for CMH services was completed for Dakota County. Referral for Red River Valley Juvenile Center has been completed and submitted. Crow Wing County Social Worker is bringing this case to her team to see if they can assist in coordination with Dakota county to work on placement. No high acuity beds available for inpatient psych placement at this time. Awaiting return call from Sanford Thief River. Pt is on the wait list at Miller Dwan in Duluth.

10/24/24:
Discharge Plan: 

Establish Residential Treatment (Recommendation made last hospitalization on 8/8/24), Mower County discussing alternative placement with 24-hour supervision. 
 RTC Referrals: 
-Avanti Center for Girls; patient unable to attend due to funding source, could attend if funding was supported by Mower County
-can send more, pending funding or alternative placement through Mower County 
Continue with established outpatient providers: 
-School-based therapy 
-PCP: Mayo Clinic Austin 
-MM: Mayo Albert Lea 
-Voluntary CPS: Mower County 
-PO: Mower County 
Estimated length of stay: 
Seven to ten treatment days, or until patient stabilizes and aftercare is established.
