CASII score updated to a 21. Patient has not had any aggression, or elopement attempts since his first week of admission back in January. He is calm, playful, and pleasant. He is loving and attentive to infant sibling. He has now been denied from Grafton, Bar None, Leo Hoffman, Chileda, Gerard Academy, and Companion Home.
At this point, recommendations are no longer for residential. He requires placement in a community group home as his parents are not able to provide him the supervision he requires. He was referred to the DHS Complex Transition Team. Currently only has a WRAP social worker involved. Any assistance or recommendations for our care navigation team to assist with placement outside of the hospital is greatly appreciated.

received end of day yesterday, needing the DA to have an addendum of recommendation for our level of care as a medical necessity of a Residential/PRTF for treatment by that provider. Hoffmann is waiting to hear back

Patient has been denied from Chileda, Bar None, and Grafton. He is doing very well on the unit with no behavioral dysregulation, elopement attempts, or aggression. He is a very kind, pleasant, young man that just needs a chance. He does very well with structure and does not do well in his current home environment.

Continues to board in the ED. DHS approved ICPC and plan was to discharge to family in WI 2/4, however family now said they can not take youth until 2/16. Continuing to look for a short term option in the interim.
