Patient was discharged home 1/22/26 and returned to the ER several hours later after reported drinking cleaning products.

We were told 1/20 that waiver options, CRS and crisis respite, are all being denied by the County. And pt is going to be unable to return to the Wingspan home, that bed went to someone else. Unsure what potential options there are for this youth at this point.

Still awaiting confirmed admission date for facilities. Patient has been stable with no aggressive behaviors or elopement attempts

County screening meeting approved 35 day assessment verses residential or crisis bed. Anticipated 4-5 month wait. Looking at discharging home with community supports while awaiting admission for 35 day assessment

Patient accepted to Mille Lacs Academy with admission date of 1/23/26

Has a potential plan in the next week or so.

Patient discharged to mom’s custody via court order on 1/10/26.

This youth remains in our ED. Continues to be on the list for the new Nexus Youth Stabilization Center but has not been accepted. All therapeutic GH options have denied and higher LOC is not being recommended currently. Had MNchoices while in the ED, searching for waiver placement but were recently told that any crisis respite is not an option.

Update 12/11: 

12/11/25 — Pending admission to CABHH, accepted on waitlist, pending if he needs his own unit there or can be on general unit, waitlist unknown. Continues to have high acuity needs and on a 1:1.
